Welcome to the DuPage County Health Departments Vital Records request page. We hope you find this service easy, and convenient to use.

The DuPage County Health Department Vital Records office hours are Monday through Friday from 8:00 a.m. to 4:00 p.m.; we are closed for all Court Observed Holidays.

Who can request the birth certificate-

  1. The person on the certificate of legal age (18)
  2. A parent listed on the certificate
  3. A legal representative or court order

Requesting a certified copy of a vital record is easy. Just follow the steps below:

Certificates will be sent via first class mail.

  1. The link below will take you to the page you  can select and pay for your certificates.
  2. Complete the required information on the screen that follows.  You will receive confirmation number once the transaction is successful.
  3. Print the confirmation page.
  4. Make a photocopy (front and back) of your photo identification. Sign and date the confirmation page. Fax your identification and the confirmation page to the DuPage County Health Department Vital Records Office at (630) 510-5411 or e-mail to

The DuPage County Health Department can only accept requests for records of events which occurred in DuPage County.  Under Illinois State Law (410 ILCS535) only specific individuals have legal access to birth and death recordsRefunds will not be provided for any purchases unless we are unable to process your order. Please note, an acceptable form of ID is required and must be provided within 30 days of processing payment. Fees remitted for records without receipt of an acceptable form of ID within this 30 day period will also not be refunded.
