In partnership with the DuPage County Sheriff’s Office, our Re-Entry Program Manager provides coordination of care for those in the correctional facility struggling with mental health and/or substance use. Goals of the program includes:
Early identification of persons with mental illness and/or substance use.
Consultation and coordination of care with the correctional facility staff, community treatment providers, and other criminal justice partners (i.e., mental health court, drug court, etc).
Assistance with insurance benefits.
Linkages to treatment upon release to providers closest to a person’s residence.
At times, we can arrange for a step-down upon release to our short-term crisis program. We aim to help ensure a stable transition from the correctional facility back into the community.
