In partnership with the DuPage County Sheriff’s Office, our Re-Entry Program Manager provides coordination of care for those in the correctional facility struggling with mental health and/or substance use. Goals of the program includes:
- Early identification of persons with mental illness and/or substance use.
- Consultation and coordination of care with the correctional facility staff, community treatment providers, and other criminal justice partners (i.e., mental health court, drug court, etc).
- Assistance with insurance benefits.
- Linkages to treatment upon release to providers closest to a person’s residence.
At times, we can arrange for a step-down upon release to our short-term crisis program. We aim to help ensure a stable transition from the correctional facility back into the community.
