WIC Appointments During COVID-19

Currently, you can enroll in WIC without going to a clinic. WIC families have an option to do phone-based WIC appointments, and complete online nutrition education.

If you have a scheduled WIC appointment, the WIC staff will call you within 3 business days of your appointment.  You will be given the option of completing your WIC visit over the phone.  During your phone-based appointment the WIC staff will schedule a curbside pick-up for your EBT card..

  • Pregnant, breastfeeding, or postpartum women, infant, and children under 5 years of age.
  • Live in Illinois
  • Have a family income at 185% or less of the Federal poverty level or receive SNAP, Title 19 Medicaid or TANF.
  • *WIC does not require proof of citizenship or status*

Intake Forms 

The documents listed below are typically reviewed in person during the first appointment. If you are new to the WIC program you will be asked to review these documents and provide verbal consent before your phone appointment. 

At your first in-person appointment, you’ll be asked to provide your signature. You will have the chance to ask any questions.
