Illinois is checking to see if you are still eligible for Medicaid. Here's what you need to do now:
Click Manage My Case at abe.illinois.gov to:
- Verify your mailing address under "Contact Us."
- Find your due date (the "redetermination" date) in your "Benefit Details"
Watch your mail and complete your renewal right away. If you need assistance, please contact us at 630-682-7400 to schedule an appointment with one of our benefitting team members.
SNAP BENEFITS:

The DuPage County Health Department can help you apply for the following benefits:
All Kids
Complete health insurance for your child and family is available. Eligibility requirements apply.
To apply, you will need to bring the following documents to your appointment:
- Social Security card(s) or number(s)
- A valid photo ID, birth certificate or US Passport
- If you are not a US Citizen, bring your LPR (lawful permanent resident) documents
- Proof of Illinois residency
- Proof of income for 90 days for all types of earned and unearned income (wages, self-employment, Social Security, pensions, unemployment, etc.)
Medicaid
Complete health and dental insurance for adults ages 19 to 64 is available. Eligibility requirements apply.
To apply, you will need to bring the following documents to your appointment:
- Social Security card(s) or number(s)
- A valid photo ID, birth certificate or US Passport
- If you are not a US Citizen, bring your LPR (lawful permanent resident) documents
- Proof of Illinois residency
- Proof of income for 90 days for all types of earned and unearned income (wages, self-employment, Social Security, pensions, unemployment, etc.)

SNAP (Supplemental Nutrition Assistance Program)
Illinois’ Supplemental Nutrition Assistance Program provides food and other necessities to low-income families. Eligibility requirements apply.
- To apply, you will need to bring the following documents to your appointment:
- Social Security card(s) or number(s)
- A valid photo ID, birth certificate or US Passport
- If you are not a US Citizen, bring your LPR (lawful permanent resident) documents
- Proof of Illinois residency
- Proof of income for the last 90 days for all types of earned and unearned income (wages, self-employment, Social Security, pensions, unemployment, etc.)
Access DuPage
Access DuPage is a program to help persons in DuPage County who are without immigration status receive primary care medical services. Eligibility requirements apply.
To apply, you will need to bring the following documents to your appointment:
- Proof of income for the last 90 days
- Proof of address
You will also be asked to complete the following forms:
- Child support verification
- Release
- Address verification

To schedule an appointment with a Community Health Worker (CHW), call 630-682-7400, option 2, option 7 and ask for a benefiting appointment. Appointments are offered at all of our Public Health Centers.
