Family Case Management

Family Case Management (FCM) is a free program that serves low-income pregnant women and infants up to age 1 who live in DuPage County. The program provides guidance and education for a healthy pregnancy, and a healthy infant through both clinic and home visits.

Our dedicated FCM Public Health Nurses and Case Managers can see you in the office and in your home. They provide information related to prenatal needs, infant care and parenting. FCM services are integrated with those provided through the Women, Infants, and Children (WIC) nutrition program.

Services

The FCM Program can help with:

  • Applying for health insurance
  • Finding a doctor for your prenatal or children's care
  • Offering screenings for infant development
  • Providing immunizations
  • Providing information about health topics including:
    • Breastfeeding
    • Infant growth and development
    • Labor and childbirth
    • Nutrition
    • Parenting
    • Well child care
  • Referrals to services available in the community
