Death Certificate

To order a death certificate, an individual must have died within DuPage County.

Who Can Order a Death Certificate

  • A duly authorized agent having personal or property interest in the records
  • An immediate relative

Fees

  • A $18 fee will be charged for the first certified copy, and a fee of $6 will be charged for each additional certified copy.
  • A $3 shipping and handling fee will be charged for mailed certificates.

How to Order a Death Certificate

You can order a death certificate online, by mail or in person.

You must have valid ID to order a death certificate. Acceptable forms of ID can be found on our acceptable forms of ID list (PDF).

Order Online

  1. If paying online, use the one time payment option on ePay and complete the required information. Do not create an account.
    • Category: Vital Records
    • Payment Type: Birth Certificate
  2. Print your payment receipt from ePay.
  3. Sign and date the payment receipt.
  4. Make a photocopy (front and back) of the requestor’s acceptable form of ID (PDF).
  5. The requested documents must be sent within 7 days of payment via email: [email protected] or Fax: 630-510-5411.

Online orders are mailed via first-class mail.
