Death Certificate

To order a death certificate, an individual must have died within DuPage County.

Who Can Order a Death Certificate

  • A duly authorized agent having personal or property interest in the records
  • An immediate relative

Fees

  • A $18 fee will be charged for the first certified copy, and a fee of $6 will be charged for each additional certified copy.
  • A $3 postage fee will be charged for mailed certificates.

How to Order a Death Certificate

You can order a death certificate online, by mail or in person.

You must have valid ID to order a death certificate. Acceptable forms of ID can be found on our acceptable forms of ID list (PDF).

Order Online

  1. If paying online, use JetPay and complete the required information.
  2. Print your confirmation page.
  3. Sign and date the confirmation page.
  4. Make a photocopy (front and back) of the requestor's acceptable form of ID (PDF).
  5. Fax or email your ID and the confirmation page to the DuPage County Health Department Vital Records Office:
