Welcome to the DuPage County Vital Records department.

Vital Records maintains records for births and deaths that occurred in DuPage County. We have access to DuPage County birth and death records beginning in 1953. Certificates may be requested online, by mail, or in person at one of our four locations, depending on the date of birth or death. Please call 630-682-7400 for more information.

Fees

Fees for certificates are due with your order form. Refunds will not be provided for any purchases unless we are unable to process your order.

Additional Information

The DuPage County Health Department can only accept requests for records of events that occurred in DuPage County. Under Illinois State Law (410 ILCS535) only specific individuals have legal access to birth and death records.

Birth and Death certificates are not public record. Records are only available upon written request from the person eligible to obtain. Phone inquiries are not available.

Please note, if ordering online, an acceptable form of ID (PDF) is required and must be provided within 30 days of processing payment. Fees remitted for records without receipt of an acceptable form of ID within this 30-day period will also not be refunded.
