Prepare for Your Dental Appointment

Please bring the following to your dental appointment:

  • A list of the medications you are currently taking, including name(s) and dosage(s)
  • Proof of identification
    • A valid photo ID, birth certificate or U.S. Passport
  • Proof of insurance (if applicable) or,
    • Your Medicaid or Managed Care card
  • Proof of income. Examples include:
    • Child support
    • Pensions
    • Social Security card
    • Statements of wages
    • Unemployment
  • If no income: Proof of income (90 days) for the person(s) helping you financially and a letter of support written by them.
  • If self-employed: Recent Federal Tax Returns and three months recent bank statements showing deposits of income.
  • If paid in cash: three months of bank statements showing the deposits of income. If you file taxes, you may also use them with the employer letter.
  • Proof of address. Please provide one of the following:
    • A utility bill, must be issued within the last 30 days, and one of the following:
      • Electric
      • Gas
      • Water
    • Current lease or proof of mortgage.
    • If you live with someone else and have none of the documents listed above to prove your address, please take the following actions:
      • Provide a letter from the person you live with indicating you reside with him or her.
      • Have the person writing the letter provide one of the above proof of address documents in his or her name.

You may save time at your visit by printing and filling out the Health History - English (PDF) or Health History - Spanish (PDF) and bring it with you to your appointment.
