Permit Applications and Forms
All Food Facility Owners
Invoices will begin to be mailed out the week of March 22nd, 2021 and payments are due on or before April 30, 2021. DuPage County Health Department understands the challenges and the additional costs associated with operating during COVID-19. In order to provide some assistance at this time, the cost for all annual food permits fees have been reduced by 50% for this permit year May 1,2021 through April 30, 2022. Annual food permit certificates will be going out starting in mid-May. Please refer to your ePay receipt or bank statement for deposited checks as confirmation of payment received while waiting for your permit.
All Pool Facility Operators
2021 annual pool permits will start on May 15, 2021 with an expiration date of May 14, 2022. The invoices will be mailed out the week of April 5, 2021, and if you have not received your invoice within two after that date please contact DCHD Billing Department at 630-682-7400, extension 2299 or email [email protected]. Online Payment Fees
Credit card transactions will be charged a processing fee of 2.25 percent or $1.00 minimum
E-check payments will be charged a processing fee of $0.75.
Please note: Processing fees are charged by the State of Illinois’ processing company and are not collected by the DuPage County Health Department.
