Permit Applications and Forms
Due to COVID-19, the DuPage County Health Department has suspended all temporary food permit applications until further notice.

Due to public health concerns and response activities, we will only be accepting online applications and payments at this time. Thank you for your understanding.
All Food Facility Owners
Invoices were sent out the week of May 15, 2020, and payments are due on or before June 30, 2020. Your current food permit expiration date has been extended until June 30, 2020. Please continue to visit the website for important updates.
All Pool Facility Operators
Your current pool permit expiration date has been extended to July 31, 2020. Invoices were mailed June 18th, 2020. If you have not received your invoice by the end of the month, contact the DHCD Billing Department. Please continue to refer to the website for new operational and public safety guidelines for your pool.
All Mobile Food Vendors
Your current Mobile Food Vendor permit expiration date has been extended to July 31, 2020. Invoices will be going out by June 19th, 2020. Please refer to the documents provided with the invoice for instructions on how to obtain your annual permit sticker.

Online Payment Fees
Credit card transactions will be charged a processing fee of 2.25 percent or $1.00 minimum
E-check payments will be charged a processing fee of $0.75.
Please note: Processing fees are charged by the State of Illinois’ processing company and are not collected by the DuPage County Health Department.
