The Population Health Unit is responsible for analyzing and reporting on health factors and outcomes to identify health needs and priorities of DuPage County residents. The Unit supports initiatives developed by the DuPage County Health Department to reduce health disparities and improve population health outcomes.
The core functions of the Population Health Unit include:
- Providing backbone support to Impact DuPage, a partnership of community leaders committed to creating a common understanding of community needs.
- Developing a Community Health Assessment and Community Health Improvement Plan (CHA/CHIP) every three years.
- Providing support to staff and community members by responding to requests for data, statistics and other public health information.
