Appointment Reminders
To help maintain social distancing in our waiting areas, please check in and complete your health screening 5 minutes prior to your scheduled appointment. If you are feeling sick or have been in contact with anyone experiencing COVID-19 related symptoms, please reschedule your appointment.
To help reduce the spread of COVID-19, patients (over the age of 2 yrs.) must wear a cloth face covering to their appointment.
In order to maintain social distancing recommendations, only one parent or legal guardian and the child scheduled to receive the immunizations will be allowed into the building.
We are implementing a new reminder call system in December. Please make sure your contact information is up-to-date to ensure you get reminder notifications.
