Community Support Team (CST)

The Community Support Team (CST) provides service to individuals with mental health needs that respond to an assertive and intensive team-based approach.

CST is a service model that provides intensive community-based support to individuals who may not progress with office-based appointments or infrequent community-based interventions.

A team approach is used to support individuals with all areas of functioning with the intention of reducing crises, connecting with community resources and engaging individuals while promoting harm reduction.

Eligibility

To be eligible for the CST program, clients must be assessed to need this level of service. The CST program is designed to help individuals with some of the following identified issues:

  • Chronic homelessness
  • Contact with the legal system
  • History of multiple crises for behavioral health-related issues
  • Medical complexity
  • Substance use
