
Family Case Management (FCM) services provide families, who meet DuPage residency and the program income guidelines, and who are either 1) pregnant or 2) have a child who is aged one or less. These families are linked to both FCM and WIC programs. Women who identify that they are pregnant are encouraged to contact DCHD to schedule a WIC appointment where they will be enrolled in the Medicaid of ALL KIDS public insurance coverage for their pregnancy. Pregnant women will also receive assistance in locating a prenatal care provider. During their program participation, women will receive health education in areas that contribute to a healthy pregnancy, such as nutrition, fetal growth and development, and postpartum depression.
Families with infants who are aged one year or less are enrolled in FCM and WIC programs, and are assessed for access to a primary care provider and enrolled in Medicaid ALL KIDS public insurance program as needed. These families receive on-going assessment and education in the areas of health care, immunizations, child growth and development, and child safety.
All families enrolled in the FCM program are assigned to a public health nurse or qualified case manager to fully assess the areas that impact their overall health, including food, housing, transportation, and family support. Information and referrals to community resources are provided to all families.
As a certified public health department in the State of Illinois, the high-risk infant follow-up nursing services are a mandatory service provided by the DCHD. Infants identified at the time of birth as having specific congenital or genetic or health-related conditions that are outlined under Illinois statute are offered services by a public health nurse. The services could include assistance with medical services liaison, developmental assessments, ongoing education, consultation and evaluation. These services are provided to any DuPage County family who has a baby that meets the health guidelines which have been established through the Illinois Department of Public Health, with no income eligibility requirement. Public health nurses can provide in-home or clinic-based visits to families in this program through the child's second birthday.
