Physical health care services for foster
children
Foster parents are responsible for:
- Obtaining an initial health screening (IF it
is not completed BEFORE placement)
- Selecting a primary care physician
- Scheduling a comprehensive health evaluation
- Obtaining routine medical examinations,
including immunizations
- Making the child's Health Passport available
to ALL medical providers at each health visit
- Bringing the child's Health Passport to each
Administrative Case Review
- Taking children for follow-up medical
appointments or referrals for further medical assessment, treatment or
services
- Informing the child's caseworker, of medical
problems or the need for medical services, as they occur
Foster Parents can call the DuPage County Health
Department at (630)682-7979 ext.7098 to obtain referrals for needed medical
services, and have access questions answered.
Foster Parents must contact DuPage County Health Department on the same day a
child in to DCFS custody is placed in their home. The purpose of the call is to
select a primary care physician in order to schedule a Comprehensive Health
Evaluation WITHIN 21 DAYS OF TEMPORARY CUSTODY. The child's caseworker must
provide the foster parent with an Encounter Form and Medical Consent to take to
the Comprehensive Health Evaluation.
