Birth Certificates

Certified copies of birth certificates for persons born in DuPage County,
Illinois may be obtained through the DuPage County Health Department.
These records may be requested by and released to the mother, father (if listed
on certificate), person named on certificate if over 18 years old, and legal
guardian or representative with supporting court documents. Certified copies may
be obtained either in person at the Wheaton office or through the mail.
The following information is required:

Date of birth:
Hospital:
Father's full name:
Mother's full maiden name:
Photocopy of valid state driver's license or state identification:
Signature of requesting party:
Relationship to child:
If you desire a certified copy of a birth certificate, please provide the above
information and bring or send with $10.00 for the first copy and $2.00 for
each additional copy for the same person to:
