Food Service Sanitation Manager Certification Refresher Course
~Complies with Illinois Department of Public Health Requirements~
Effective October 1, 1999, all Illinois Certified Food Service Sanitation Managers must complete a Food Sanitation Refresher Course or exam in order to renew their current certificate. To help our local food service operators comply with this new law, the DuPage County Health Department will be offering a State approved refresher course at the following dates & times in 2009.
COURSE SCHEDULE:

Course 01: Tuesday, February 24, 2009, 9:00AM – 3:30PM

Course 02: Tuesday, May 19 and Wednesday, May 20, 2009, 5:30 – 8:30PM

Course 03: Wednesday, October 14, 2009, 9:00AM – 3:30PM

DUPAGE COUNTY HEALTH DEPARTMENT
111 North County Farm Road, Wheaton, IL 60187, Lower Level Conference Room #1
COURSE CONTENT & FORMAT:
Topics of discussion will be foodborne illness prevention, personal hygiene, cross contamination, cleaning & sanitizing, HACCP and emerging food safety issues. The format will include videotapes, lecture, demonstrations, and group activities.

Complete the registration form and send it with check payment of $65 at least one week prior to the course date. Registration is limited to 20 students per course.
· PARTICIPANTS MUST ATTEND THE ENTIRE SESSION TO BE ELIGIBLE FOR
REFRESHER COURSE CREDIT.
· PARTICIPANTS MUST PROVIDE A COPY OF THEIR CURRENT ILLINOIS FOOD SERVICE MANAGER’S CERTIFICATE AND A PHOTO ID AT COURSE SIGN–IN.
