DCHD AWARDED GRANT TO INTEGRATE MEDICAL CARE AND BEHAVIORAL HEALTH

DUPAGE COUNTY-The DuPage County Health Department (DCHD) announces that it has been awarded a nearly $40,000 grant to begin coordinating behavioral health and medical care by employing new solutions in health information exchange.
 
The DCHD grant was one of six innovative community projects awarded to advance Illinois' efforts to integrate behavioral health and medical care. The DCHD grant was announced by the Illinois Health Information Exchange (ILHIE). The grant is funded under the Illinois Behavioral Health Integration Project (BHIP), hosted by the Office of Health Information Technology.

Health information exchange will allow for the safe and secure transfer of medical and behavioral health records. In Illinois the ILHIE serves as a transport network that transmits health records from one provider to the next. This exchange of health information is an important safeguard in assuring that healthcare providers have timely access to all of an individual's pertinent health information, not just the records generated in one provider's system.
 
DCHD, a licensed Illinois behavioral health provider, and VNA Health Care (VNA), a community-based medical provider, are working together to integrate behavioral health and primary care services for adults with serious mental illnesses (SMI). DCHD and VNA plan to use ILHIE to share pertinent behavioral health and medical information about individuals who receive care from both agencies.
 
According to the Substance Abuse and Mental Health Services Administration (SAMHSA), 68 percent of individuals with a mental health disorder have one or more medical conditions. Of the nearly 60 percent of adults with a medical condition, about one-third have a co-occurring mental disorder. Those with co-occurring mental health disorders and chronic medical conditions experience much poorer health outcomes.

The BHIP project will help DCHD and VNA take an important first step in the implementation of integrated behavioral health and primary care services.
