
Illinois Project for Local Assessment of Needs (IPLAN) is a process developed by the Illinois Department of Public Health (IDPH) to meet the requirements set forth in 77 ILL. Adm. Code 600 that includes a series of planning activities conducted within the local health department (LHD) jurisdiction. Each LHD jurisdiction shall recertify every five years, and required deliverables include a community health needs assessment and a community health plan. IDPH approval of these documents establishes the LHD as a 'Certified Local Health Department.'

The Administrative Code allows the LHD to use an equivalent planning process known as Mobilizing for Action through Planning and Partnership (MAPP) to meet IPLAN requirements. MAPP is a strategic approach to community health improvement and focuses on the local public health system, broad community and public health leadership participation, the use of data to inform decisions, and the use of partnerships and collaborations to optimize resources, responsibility, and performance.
In August 2010, the DuPage County Health Department completed the 2015 IPLAN MAPP planning process. The six MAPP Phases are listed below, along with the IPLAN 2015 Community Health Plan.
