SURVEY GOALS

To obtain information on health behaviors that may lead to disease, disability and premature death.

To provide baseline and comparative information against which to measure future progress in meeting health goals for the community.

To meet certification requirements under IPLAN (Illinois Project for Local Assessment of Needs).

To identify sub-populations and health problems that may be addressed by the Health Department, hospitals and other local agencies.
