1. Who can get a birth certificate?

A mother or father (if listed on certificate), the person if 18 or older or a legal guardian/representative (with supporting documentation).

2. Can you take my order over the phone and then fax certificate?

Orders are only taken in person or through the mail because payment (check or cash only) must accompany the request. Certificates cannot be faxed because they would not be a legal document. The raised seal must be evident for the certificate to be legal.


3. What is the cost?

The cost is $10.00 for the first copy, $2.00 for each additional copy for the same person if requested at the same time.

4. Do you take charge cards?

 We accept Visa, MasterCard, American Express, or Discover

5. Who can get a death certificate?

Death certificates may be obtained for the following reasons:
   1. Genealogical purposes if the death certificate has been on file at least
       20 years
   2. To persons having a personal or property right interest
   3. Upon a court order from a competent jurisdiction
   4. Executor or administrator of decedent's estate

The cost is $12.00 for the first copy, $4.00 for each additional copy for the same person if requested at the same time.

6. What are your hours?

Monday through Friday, 8:00 a.m.- 4:30 p.m.

7. Is this the only place where I can get a birth or death certificate?

Certified copies of birth and death certificates are available in the Wheaton office of the DuPage County Health Department, 111 N. County Farm Road. Copies are also available from the State of Illinois or the DuPage County Clerk.
