The Expert Committee Recommendations Regarding the Prevention, Assessment, and Treatment of Child and Adolescent Overweight and Obesity: Summary Report was published in Pediatrics in 2007. This document outlines the recommended actions physicians should perform with their patients to prevent, assess, and treat childhood obesity.  To develop the Pediatric Obesity Protocol, FORWARD reviewed the Summary Report along with other documents to develop a step-by-step guide to the prevention, assessment, and treatment of overweight and obese children.

To support healthcare providers with their need to assess lifestyle habits and readiness to change (Action Steps 8 and 9 in the Protocol), FORWARD compiled a nine-question survey, available in English and Spanish, that can be administered to patients or their caregivers.  The Patient Lifestyle and Readiness to Change Survey can be found via the links below:
