This chart lists the conditions that MUST be reported to DuPage County Health Department in accordance with Illinois law, and the time frame in which reporting must occur: immediately, within 24 hours, or within 7 days. Effective March 3, 2008, each case of Chickenpox (Varicella) is now reportable within 24 hours of receipt of notification from a parent, guardian, or health care provider. The Chickenpox Case Report Form requests information that is to be collected and reported to DuPage County Health Department. Please complete the entire form and fax to 630-510-8923.
