Birth Certificates

Certified copies of birth certificates for persons born in DuPage County, Illinois may be obtained through the DuPage County Health Department.

These records may be requested by and released to the mother, father, co-parent (if listed on certificate), person named on certificate if over 18 years old, and legal guardian or representative with supporting court documents. Certified copies may be obtained in person at the Wheaton office, through the mail (using form obtained on the Internet) or request and pay via the Health Department website. All requests that require mailing will be charged an additional $2.95 for postage and handling.

If you would like a certified copy of a birth certificate, please provide the above information and bring or send $14.00 for the first copy and $2.00 for each additional copy of the same record.
