will ask you some questions to clarify your situation. If our services do not
match your needs you will be given resources that will be of benefit to you.

- How can I get a grant to place my child in
residential services?
Call Department of Human Services Office of Mental Health at
773-794-4886 and request an individual Care Grant application
and information about services.
- How can I get in touch with other people
who have a mentally ill family member?
